GUIDE FOR NEW PATIENTS
Have your appointment in one of our clinics or, if you are a resident of Ontario, you can choose to be seen through our online video call service, Home Care.
The below guide is for our in clinic appointments.
ASK YOUR DOCTOR TO REFER YOU
Have a physician complete a Physician Referral Form. If a physician referral is not attainable, a Self-Referral Form is available for patients over the age of 25. Please ensure to download the form you need first. You can then choose to open from your computer and fill in the PDF or print and complete by hand.
After all the required medical history and treatment information is received from your doctor, we will contact you to schedule an appointment for your assessment.
CONSULTATION AND ASSESSMENT
We advise to set aside approx. 2 hours for your entire visit. We will review your documents and do a quick physical before you meet with the doctor who is there to assess and potentially authorize the medical document for cannabis.
COUNSELLING AND GUIDANCE
If medical cannabis is authorized, the onsite counsellor will advise you about cannabis use, assist with strain selection and registration with the best matched licensed producer from whom you will order the cannabis from.
Regular follow-ups are arranged to track your progress. A new cannabis medical document will be issued as needed.
There are no fees to be a patient in our clinics. If you are a new patient and wish to have an online video call appointment,
there is a small fee of $19.99
